  1. Guarantee letter only for companies that registered with PPUM.

    2. Guarantee letter from student health center/student clinic (Applicable for IPTAs student only).
  5. SOCSO cases (only conform case) - Form 21 that certified originally by SOCSO officer.
  6. Disable Patient - OKU registration card issued by Jabatan Kebajikan Masyarakat.
  7. Welfare - Bank book or other document stated as monthly welfare receiver.
  8. PPUM Retiree - Entitlement retiree card issued out by PPUM Human Resource (Red, Yellow and Green)

Payment

  1. Cash - Only at clinics payment counter.
  2. Credit Card - Visa, Master and American Express
    (Please proceed payment at Unit Hasil Payment Counter, Ground Floor Menara Utama)

*Any payment to claim back from company or insurance please make payment at Unit Hasil Payment Counter, Ground Floor Menara Utama.

Refund

  1. Document needed:-
    1. Registration card (Blue Card)
    2. Original payment receipts.
    3. Identification card.
    4. Proxy letter if the claimant name differs from the payer/patient in the receipt.
    5. Memo/letters from the doctors if necessary.
    6. Supporting document such as guarantee letter, pension card or school letter.
  2. Refund will be paid in cash.
